In order to provide you better care, we participate in a variety of programs with Medicare. Here are some that you may want to know about:

Initial Preventative Physical Exam

In order to best serve you, we want to get to know you better. We’ll talk about your health history, as well as your family and your social health histories. We’ll also talk about what preventative measures you can take to lead a healthier life.

 

  • Medicare Part B Enrollees: Medicare Part B only covers one IPPE. You must do the visit within 12 months of enrolling in Medicare Part B.
  •  

  • There is no out-of-pocket cost for this visit.

Medicare Annual Wellness Visit

Things change, and sometimes health plans need to adapt. These annual Medicare visits are used to check in on how your personalized prevention plan is working and to perform an annual health risk assessment.

 

  • Medicare Part B Enrollees: Medicare Part B covers one Wellness Visit every 12 months.
  •  

  • There is no out-of-pocket cost for this visit.

By setting us as your Primary Clinician, we’re able to help you make the right decisions for your care needs.

Primary Care First

Primary Care First (or PCF) is an innovative new program currently being piloted by Medicare. It changes the way we work with Medicare and allows us to provide you a higher standard of care.
